The first extension (at issue here) is an extension of the classic Carcassonne game in blue carton. It includes: 18 new land tiles, 8 followers for the 6th player, 6 large followers, 6 points cards (a 50/100 points)
My opinion on this and the other enhancements:

This first expansion offers several new game-platelet and especially the expansion males for 6 players! You would not be necessary if you already want to play with a maximum of 5 persons. With 6 players a game is very short, if "only" playing with the first enlargement!
Remember! Multiplayer, more land tiles (tiles) needed!
If you are on more gameplay from which additional platelets are these (and
each other!) extension anyway a must. For fans of the game anyway.
Of particular importance is the 2nd expansion -> "dealers and builders" (dark red box). Thus, it will be a bit more complicated and tricky. But also makes all the more fun and you need a little more playing surface. The (new) goods platelets are counted only at the end of the game. Paths on which (with the new card) is an inn count twice, but only if they have been completed. There are interesting new tactics and even the builder can one help during the game more points to grab. I think these 2 extension for the most interesting and no longer want to miss it. [In the second extension also a blue cloth bag (called Le Sac) where in the game you can do all platelets to them "completely" blind drawing ... looks nice is more practical than eg "The Tower" -Kartenspender! ].
The (mini-) extension "King and scout" is not exactly the rage, because it comes into play only at the end of the game and one insane must be careful who built the biggest castle, or the longest road. Let's play with this expansion rarely, if ever. The handful of new land tiles are of course welcome!
The expansion "The River" was first only appendages of the PC game Varainte (on CD) of Koch Media. Really I can not recommend this extension. It is not really necessary; perhaps only if one but has a larger play area, a few more cards and a slightly longer duration of game. But this is nothing and you have to keep the few cards always good separately and "build" in the beginning especially tricky. Commercially is incidentally also "The River 2", which you can also bid low. Something for hardcore Carcassonn-ists.
The extension "Count of Carcassonne" offers new opportunities neat to pull his teammates on the table. The few new cards with the new "Counts of Carcassonne", however, are not absolutely necessary and useful only for die-hard fans. The new cards are laid out at the beginning of the game and complicate the rules really powerful. In addition, this extension is not "flow" can be combined. A combination with "damsel and dragon" proves to the game as too complicated. It is at the "Count" after thorough consideration to one of the most unnecessary expansions.
The 3rd (official) extension called "damsel and dragon" is used to make the game more unpredictable and confuse the players completely, or to steal. You can see the new land tiles however involve into play without the kite and let the wandering damsels. ;-) New challenges and because of a complicated set of rules, if you will, a change; foresighted tactics is nearly impossible.
The latest and 4th at the moment extension "Carcassonne, The Tower" also provides new tiles, but also pieces (for the tower) and the card dispenser Tower. The Tower (on designated new platelets) is new and first - like anything new - interesting but makes maneuvering very complicated, and the original appeal of the game is made mE naught. Anyone who has the human Ägere-you-Not around cries when he flies out permanently, will have the tower (an opponent) fortiori his "fun"; or not.
The card dispenser Tower I think is a nice idea, but totally useless if, for example with more than 4 people playing at the same table ... where to put the tower there, so all tuned come good? Better to "Le Sac", the blue cloth bag!

for extreme-Carcassonne-ists: The magazine "Spielbox" has published a special edition published "Carcassonne Almanac incl extension The Cathars."
(You can also order!). In the booklet there are heaps of information / opinions / interviews for this game. In addition, some new tile, entitled "The Cathars". The extension is for Punkteklau but extreme Carcassonne fans - like me: -) - will have fun at every new tile; as long as it has the same good quality as the basic game!
The quality of the landscape maps that it be interpreted and applies to occupied is surprisingly good. In our family extremely-Carcassonne is played and it is remarkable how long the plates and boxes endure all this.
Playing time varies depending on how many people and with extensions to play. But 30 minutes minimum.
Tip: Who plays Carcassonne with all its extensions, or with double base and extension games, which should have a large play area for it. But you'll probably notice even very fast ... ;-)
Tip: There are many other "Carcassonne" - variants, all of which have their own charm. I have grown a lot as the "hunter-gatherers" to his heart; who, however, nothing to do with this game here. Or "The Castle" you play just the two and where there is no need for extensions.
