The gameplay remains the same even with this extension:
Monster create bases, play actions and use abilities / effects. For destroyed bases there VPs. Whoever reaches 15 points first wins.

Here beats to the madness. And that is an interesting innovation. "The final Cthulhu" introduces new types of cards: cards madness. Various actions in the game, the player must draw these cards to certain events. You will be added to your hand and treated like normal action cards. After use they wander on your discard pile.
The important thing. For every 2 tickets madness end of the game in your deck, the player gets -1 victory point.
Example: Player A has 15 victory points, the game is over. However, he has 6 madness cards in the deck. 3 Victory Points are therefore deducted. The game is still closed, but Player A has only 12 effective VPs. If another player has more wins this.
The principle is well thought out and is cleverly used by the new fractions partly (because madness cards can also be removed from the deck!).
But now the disappointment:

All Madness tickets are the same. They look the same and the text is always like this: put the madness card back OR draw 2 cards and put the madness card on your discard pile.

This was absolutely disappointing because since one would have to kreiieren with a bit more creativity many great action cards.
Otherwise, there are 4 new factions (University, (fish) residents of the town of Innsmouth, henchmen Cthulus and foreign beings), all as always have their strengths and provide more variety. Fractions are interesting and in my opinion, well done, but do not come ran to the 2nd extension and make the game just a little too complicated.
So my conclusion is as follows:
The political groups and the principle of madness cards are good additions to the base game and the 1st expansion, yet disappoint due to a lack of imagination.
For the small price definitely worth a purchase, but first you should grab (9000 Wahnsinnslvl) definitely the base game and the first expansion.
