The part of the device which is clamped directly behind the ear is 1.5 cm wide at its widest point, which is not exactly little and rather unusual. I tested the unit at the other ear and there the same problems, but reduced. Other people have reported similar, while others were lucky and it fit comfortable.
The button for accepting calls is integrated into the speaker and makes him thus greater (if my guess) and precisely this round part presses on the lower part of my ear canal, even though I have this height-adjustable part already fully retracted.
